A healthcare company has a vacancy for a DISEASE MANAGEMENT MANAGER
Purpose of the Job
The Disease Management Manager leads, organizes and manages the Disease Management Programme of the Company which is both a proactive disease management service and a reactive disease management service. The purpose of the position is to control the financial and clinical risk of contracted medical schemes/health plans through the implementation of a structured Disease Management Programme ensuring the company remain compliant, efficient and profitable The Disease Management Manager will also be responsible for formulating strategy, improving performance, procuring resources and securing compliance. You should be able to mentor your team members, find ways to increase quality of customer service and implement best practices across all levels.
Duties & Responsibilities
REPORTS TO: AMBULATORY CARE MANAGER
Accountability for the function's performance, within the agreed strategic framework
Develop and retain skilled, engaged employees
Provide strategic and implementation input into stakeholder models and initiatives
Provide input into and develop the functional strategic plan
Ensure strategy is implemented in a manner that supports operational and market imperatives
Build and maintain relationships with stakeholders to support the development of mutually beneficial and supportive relationships
Ensure the short- and long-term sustainability of the division
Optimise the function's core internal processes and internal supply chain and resolve functional operating conflicts.
Drive delivery of customer expectations within organisational quality and quantity standard
Communicate strategic context and strategic implementation plans to ensure operational best practice and continuous improvement
Ensure client service levels are met.
Incorporate the customer experience strategy of the organisation and its metrics.
Desired Experience & Qualification
Experience and Qualifications
Degree or diploma in nursing or other health discipline.
Current and continued valid registration with the relevant Health professions Council.
Previous supervisory or management role an advantage.
Experience with ambulatory patients, especially patients with chronic illnesses would be an advantage.
Experience and knowledge of patient counselling in the managed care area an advantage.
Experience and knowledge of the medical scheme environment an advantage.
Experience and knowledge of the Medical Schemes Act and its Regulations, particularly pertaining to PMBs and the CMS guidelines an advantage.
Experience as patient counsellor or nurse educator an advantage.
Experience in Disease Management reporting an advantage.
Experience budgeting and forecasting
Excellent communication Skills
Outstanding organizational skills
English: Primary business language. Be able to speak, write, spell, read and comprehend the language.
Other languages: The ability to converse in another South African language e.g. Afrikaans, Zulu, Xhosa, Sotho an advantage.
All written and verbal communication must be conducted in a professional manner.
Proficiency with the following programmes is required:
MS Excel Intermediary or Advance
MS Outlook/ email
Some prior knowledge /experience with medical scheme administration and/or managed care systems.
Own transport and valid driver’s license are essential.
Travelling may be required for offsite testing and Health check days.
The hours of work: Office hours from Mon to Fri, 8am to 5pm.
Participation in regular performance appraisals is a requirement.
